FOR CLASSES HELD BEFORE 8/1/19 ONLY: Before 8/1/19, providers who successfully completed a MAT Program class received a paper certificate set, sent by standard USPS mail. If you successfully completed at MAT Program class before 8/1/19, and your certificate has been lost, this the place to order a duplicate certificate set.
If you are a MAT Trainer who needs to order a duplicate certificate set, please go to the For Trainers product category and order it there.
The duplicate certificate set includes two certificates issued in your name (unless you specify otherwise). The certificates will be delivered by standard mail.
If you are ordering duplicate certificates for someone other than yourself, please put THAT PERSON'S NAME in the First Name on Certificate and Last Name on Certificate fields.
When you get to the payment screen, enter the first and last name of the PERSON WHOSE CREDIT CARD IS BEING USED to pay for the certificates on that screen.
WARNING - you are only eligible to order a standard duplicate MAT Program certficate set for classes held before 8/1/19.
This product is NOT REFUNDABLE.
